Residential Real Estate Loan Application

The application for a Residential Home Mortgage Loan has one main application form along with separate attachments that should be completed if they are appropriate for your situation. See below for descriptions.

All applicants should begin by completing the Uniform Residential Loan Application.

If you are applying jointly with another individual (spouse, partner, parent, child, etc.), the second applicant's information needs to be provided on the Uniform Residential Loan Additional Borrower form.

The Uniform Residential Loan Continuation Sheet should be used to record any additional information that you need to provide to the bank that did not fit on the Application or Additional Borrower form.

The Uniform Residential Loan Unmarried Addendum should be used when a borrower selected "unmarried" in Section 1 of the Application and information regarding civil unions, domestic partnerships, or registered reciprocal beneficiary relationships will be necessary to determine how State property laws directly or indirectly affect creditworthiness.

When you have completed the appropriate forms, you may attach them to our secure contact form to send them to a Lender or you may print the completed forms and drop them off at the bank, or mail them to 112 W Fulton St, Waupaca, WI 54981. If you choose to send them via the secure contact form, your signature(s) on the application will be collected at a later date.
